Extended health care plans or employment assistance programs (EAP) may cover part or all of the counselling fees. It is the client's responsibility to enquire about coverage from his/her insurance provider before we begin sessions. While you are responsible for the fee at the beginning of each session, we will provide you with a receipt with our registration numbers which you may submit to your insurance company or use for income tax purposes. Healthy Directions does not deal directly with insurance companies.
If it is necessary for you to cancel an appointment, please do so 24 hours prior to your scheduled session. Appointments missed or cancelled with less than 24 hours notice will be charged $75.00. Exceptions to this include a death of someone you know or hospitalization.
All new clients need to read and sign a copy of this document before sessions can begin.
